Dental Prospect Plan

$183.00 Annual Fee

Two comprehensive exams by the Doctor in a 12 month period, two professional cleanings in a 12 month period(excludes scaling/deep cleaning), medical history review, blood pressure screening, oral cancer screening, one full set of cavity detecting xrays (Includes: 1 Panorex, 4 Bitewings, and 3 Periapical (PA) of font teeth), and 20 % off any dental treatment. (Note: This does exclude crowns, dentures, bridges & implants.)

One comprehensive oral exam, one professional cleaning, medical history review, blood pressure screening, oral cancer screening, one full set of cavity detecting xrays (Includes: 1 panorex, 4 bitewings, and 3 periapical(PA) of front teeth)

The Smile Plan membership is renewable on a yearly basis.  Membership is non-refundable and all benefits are useable only in the 12-month period after the membership is paid in full.  Care Credit and Citi Health Card do not apply.  There are no exceptions to this policy.  Since the Smile Plan entitles our members to a generous discount of 20%, we also require that all treatments be paid for in full at the time of service.
